Collect Semen Sample
How to Collect a Semen Sample for Fertility Examination (Not Post-vasectomy)
Please Note:
Check with the Sister at your local Perth Medical Laboratories Collection Centre (listed on the back of the request form) for a suitable time to collect the sample and bring it in, since the sample must be examined at the laboratory in Fremantle within 1 hour of collection.
Procedure:
(1) Abstain from ejaculating for at least 3 days immediately prior to collecting your sample.
(2) Collect the entire sample by means of masturbation only.
· DO NOT collect the sample by interruption of intercourse.
· DO NOT use a condom or lubricant as these will interfere with the test.
(3) Collect the sample directly into the provided container.
(4) After collecting the sample, replace the lid and write your full name and date of birth on the outside of the jar, as well as your doctors name.
(5) DO NOT refrigerate the sample; keep it at room temperature and deliver immediately to the collection Centre or preferably the main laboratory located at 152 High Street Fremantle.
(6) Download and Complete this information sheet and attach it to your request form.
